This form is to request BUMC IT to configure a previously purchased network printer.

This will be a number on the bottom of the port panel usually starting with a D and never a V, (V is for phones contact Telecom x86890), Most BU ports have red inserts any port with a blue insert is BMC, (Contact BMC ITS x44500)
