Addressing Perinatal Mental Health in Birthing Migrants:

The Living Well at Home Program (LWAH) functions at Boston Medical Center’s housing department. Formerly known as Elders Living at Home, the LWAH case manager’s and social worker’s work incredibly hard to stabilize both BMC patients and members of the Greater Boston community in housing. During this presentation members of the LWAH leadership team will provide a brief overview of what mental illness and medical conditions sometimes look like for Boston’s unstably housed residents and explain what the LWAH Program does and how to refer clients. We will share how stable housing has the ability to impact utilization of inpatient and emergency services.

    1. Describe the process to refer eligible patients to BMC’s Living Well at Home Program.
    2. Recognize how housing is associated with reduced behavioral health admissions.
    3. Identify common psychiatric diagnoses among individuals experiencing housing instability.
