Improving Inter-hospital Transfers

This is a poster that I presented at the Institute of Healthcare Improvement National Forum for a project I worked on at the West Roxbury VA. Multiple instances of inter-hospital cardiology transfers without adequate handoff or documentation led to the implementation of a standardized transfer accept note in the EMR.

Swetha, great idea to attack a very common problem on all of our inpatient services. Any idea how the standardized accept note has impacted the quality of care of the transferred patients? Dave
