DEAN’S LETTER REQUEST FORM, Class of 2014

The Dean’s Letters will be written from February-June of 2013. The OSA staff will write the initial drafts of your Dean’s Letters, based on your Biosketch and student file. The Student Affairs Deans will write the unique characteristics paragraph and the final paragraph of the letters, and edit the initial drafts. Dr. Angela Jackson, the Associate Dean of Student Affairs, will do a final read of all letters before signing them.

Please complete and submit the form below to request the Student Affairs Dean you would like to write your Dean’s letter, by February 4, 2013. We will make every effort to use your selection, but we also need to distribute the workload in an equitable way.

You are expected to make a one hour appointment to meet with Dean who will be writing your Dean’s Letter during the months of February-May, to discuss the letter, as well as your plans for residency.

Please be sure to make your selections based on which person knows you well, and with whom you feel most comfortable working. All have extensive experience in counseling and working with students going into every area of medicine.

  • Dr. Angela Jackson, Associate Dean of Student Affairs: Internal Medicine; General Internist; interests in medical education & health policy; former Program Director
  • Dr. Phyllis Carr, Assistant Dean of Student Affairs; Internal Medicine/Women’s Health; Councilor for AOA; GWIMS representative for BUMC
  • Dr. John Polk, Assistant Dean for Student Affairs; retired Cardiothoracic Surgeon
  • Dr. Kenneth Grundfast, Assistant Dean for Student Affairs: Chair and Professor, Department of Otolaryngology-Head and Neck Surgery
  • Dr. Karen Symes, Assistant Dean for Student Affairs; Associate Professor of Biochemistry
  • Dr. Dan Chen, Assistant Dean for Student Affairs, Assistant Professor of Medicine, Associate Medicine Clerkship Director
  • Dr. Joel Alpert: Professor of Pediatrics & Public Health emeritus: Pediatrics, Public Health; Former Chair of the Department of Pediatrics; Past President of the American Academy of Pediatrics. Dr. Alpert is also knowledgeable in Family Practice.
  • Dr. Robert Beazley: Professor of Surgery emeritus; Former Chair, Section of Oncology and Endocrinology
  • Dr. Rafael Ortega:  Associate Dean for Diversity and Multicultural Affairs; Professor and Vice-Chairman of Academic Affairs, Department of Anesthesiology.
  • Dr. Samantha Kaplan: Assistant Dean for Diversity and Multicultural Affairs; Assistant Professor of Obstetrics & Gynecology.
