Excellence in Community Engagement and Advocacy Award

The DoM APP Excellence in Community Engagement and Advocacy Award is intended to acknowledge the value of advancing meaningful partnerships and advocacy efforts that improve patient care and community well-being. APPs who have demonstrated sustained commitment to community engagement and advocacy within the past 3 years will be considered. Nominees should have made meaningful contributions to initiatives that address community needs and promote equitable healthcare access and outcomes.

Nominees with these admirable qualifications will be considered:

• Demonstrated impact in building and sustaining community partnerships
• Advocacy efforts that promote access to care and improve patient outcomes
• Engagement in outreach, education, or programming that benefits populations
  served
• Identification and response to social, structural, or systemic barriers affecting
  patient health
• Empathy and cultural humility in working with patient communities
• Professional and respectful collaboration with community partners, patients, and
  healthcare professionals
