Transcript Requests

Boston University follows the guidelines of the American Association of Collegiate Registrars and Admissions Officers (AACRAO), stating that an official university transcript reflects all academic coursework completed at the institution.

Transcript Details:

  • Transcripts provide a comprehensive record of a student’s academic progress, including credit earned by examination, transferred credit from other institutions, and degrees awarded at Boston University.
  • Grades for courses taken at other institutions will not appear on your Boston University transcript. To obtain these grades, you must contact the institution where the courses were taken.
How to Order a Transcript

Transcript Request Form

All requests for transcripts, licensure forms, and verifications for graduates of the M.D. program should be sent to the Office of the Registrar via email: camedreg@bu.edu.

You can request either an official or unofficial transcript, which includes a full record of your academic progress.

Official and Unofficial Transcripts:

  • Official Transcripts include the Registrar’s signature and the Boston University seal. They must be requested in writing and can be mailed to a designated destination.
  • Unofficial Transcripts contain the same academic information but lack the Registrar’s signature and the University seal. Unofficial transcripts can be emailed to a designated destination.

Please provide the following information with your request:

  • Full name (including any former names)
  • Signature
  • Boston University ID number or last 4 digits of your SSN
  • Date of birth
  • School attended
  • Dates of attendance
  • Degrees awarded
  • Complete address for transcript destination (or email address for unofficial transcript)
