Silver level sponsorship: $2500
Includes:
- One complimentary conference registration to the 3-day conference (AOD-SBI Meeting and INEBRIA, September 21-23, 2011)
- ¼ page add in the conference booklet
- Verbal recognition at the conference
- Space for materials/resources on an un-manned 6-foot exhibit table during the conference
Gold level sponsorship: $5000
Includes:
- Two complimentary conference registrations to the 3-day conference (AOD-SBI Meeting and INEBRIA, September 21-23, 2011)
- ½ page add in the conference booklet
- Verbal recognition at the conference
- Space for materials/resources on an un-manned 6-foot exhibit table during the conference
- The opportunity to include one item with the conference materials that are distributed to all participants
Platinum level sponsorship: $10,000
Includes:
- Three complimentary conference registrations to the 3-day conference (AOD-SBI Meeting and INEBRIA, September 21-23, 2011)
- Full page add in the conference booklet
- Verbal recognition at the conference
- The opportunity to include one item with the conference materials that are distributed to all participants
- An exhibit table (6-feet long) for materials/resources that is staffed by 1 person from your organization for the duration of the conference. The exhibit table includes a chair, waste basket, electricity and wi-fi.
Additional Information
Payment: Please mail a check made out to EVANS MEDICAL FOUNDATION with the notation “8th annual INEBRIA Conference Sponsor” on the check. Checks must be received no later than August 15, 2011, and should be mailed to the attention of Amy Alawad at the following address:

Registration: Sponsors will receive a PIN number for their complimentary conference registration(s) when payment is received and the check clears.
Additional costs: Sponsors are responsible for all costs associated with travel and accommodation. Sponsors are also responsible for all costs associated with exhibiting, including shipping/delivering materials to the conference site.

Cancellation Policy: All sponsor fees must be paid in advance (by August 15th) and are nonrefundable.
Security Requirements: Neither the conference organizers, the INEBRIA Network, nor the Liberty Hotel will assume liability for loss of merchandise or damage to sponsor’s property. The sponsor assumes the entire responsibility and liability for losses, damages, and claims arising out of injury or damage to sponsors’ displays, equipment, and other property brought on the premises of the hotel and shall indemnify and hold harmless the conference organizers, the INEBRIA Network, and the Liberty Hotel, their agents, servants, and employees from any and all such losses, damages, and claims.
