Forms for 4th-yr Student IH Electives
The following is a list of forms for fourth-year students who will undertake an IH elective.- ADD Form for IH Elective (to add an IH elective to your fourth-year schedule)
- Travel Grant Application (to apply for funds for an IH elective that will be completed in the future)
- Post-Elective Report & Travel Grant Application (to report on your elective and to apply for funds after elective was undertaken)
- Post-Elective Report (to report on your elective upon return; form must be completed by all students)
- Liability Form (all students who undertake an IH elective or summer project must print, sign, and return this form)
- On Call International Application (medical evacuation insurance for those without Aetna health plan)
