Fellow Application Form
15th Annual Fellow Immersion Training (FIT) Program: Fellow Application
To complete the application process, we require:
1) A CV.
2) A personal statement of no more than 500 words describing your motivation and goals for participation in the FIT Program. Relate your interest in addiction medicine and research to your previous training, fellowship plans, your career aspirations, and how you will apply the knowledge gained at the FIT Program.
3) A description of no more than 300 words describing a research project idea that incorporates addiction medicine and that you could undertake during your fellowship. This description should include the project aims, research question, methods and potential implications.
4) A letter of nomination from your fellowship program director that acknowledges their support for your attendance at the FIT conference, if selected, and details the percentage of time dedicated to research that you will have during your fellowship.
5) A letter of support from a faculty mentor who will assist you in incorporating addiction medicine into your clinical research. Note: if your fellowship program director is also your mentor, only one letter is required.
