BUMSR Core Library Preparation + Sequencing Service Request

Please indicate the exact name of the RNA Isolation Kit you used to prepare your samples. If you did not use a kit, please specify your process and place additional details in the notes of this form.

You can download and fill out the sample information excel sheet here before uploading it below. Please limit your sample names to at most 5 characters. Be sure to include at minimum the sample names and volumes, even if you have yet to determine the concentrations.
