The goal of DAS <span><\/span> is to provide services and support to ensure that students are able to access and participate in the opportunities available at Boston University. In keeping with this objective, students are encouraged to utilize the resources of Disability &amp; Access Services to the degree they determine necessary.<\/p>\n<h4>How to Obtain Services in GMS<\/h4>\n<p>The <span style=\"color: #0066cc;\"><a href=\"http:\/\/www.bu.edu\/disability\/\" target=\"_blank\" rel=\"noopener noreferrer\" style=\"color: #0066cc;\">Disability &amp; Access Services<\/a><\/span> office (DAS) is located on the Charles River Campus. Please complete the following to gain service.<\/p>\n<p>NOTE: This process is confidential. At no time will any student health \/ disability information be shared with GMS, your program director, or faculty.<\/p>\n<ol>\n<li>Complete the BU DAS <a href=\"https:\/\/bu-accommodate.symplicity.com\/public_accommodation\/\"><span style=\"color: #0066cc;\">Application <\/span><\/a>.<\/li>\n<li>Provide <span style=\"color: #0066cc;\"><a href=\"http:\/\/www.bu.edu\/disability\/accommodations\/all-disabilities\/\" style=\"color: #0066cc;\">documentation <\/a><\/span> to Disability &amp; Access Services by uploading (with the intake form) or emailing it as an attachment to <span style=\"color: #0066cc;\"><a href=\"mailto:access@bu.edu\" style=\"color: #0066cc;\">access@bu.edu<\/a> <\/span>in advance. Make sure you download the appropriate <span style=\"color: #0066cc;\"><a href=\"https:\/\/www.bu.edu\/disability\/accommodations\/all-disabilities\/\" style=\"color: #0066cc;\">disability verification form<\/a><\/span> if needed and have your healthcare provider complete the form.<\/li>\n<li>Make an appointment with with one of the DAS staff members on the Charles River Campus by calling 617-353-3658<\/li>\n<li>Following approval by DAS and the Dean of the School of Medicine, the student is notified of accommodations by the GMS-Disabilities Liaison.<\/li>\n<li>The student completes the <a href=\"https:\/\/www.bumc.bu.edu\/gms\/students\/dei\/gms-disability-services\/arf\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"color: #0066cc;\">GMS-Accommodation-Release-Form (ARF)<\/span><\/a><\/li>\n<li>Upon receipt of the GMS ARF, GMS-Disabilities Liaison notifies Course Directors of the accommodations<\/li>\n<\/ol>\n<p><\/div>\n<\/div>\n\n<p>&nbsp;<\/p>\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">Transferring Accommodations from BU Undergrad to GMS<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\"><\/p>\n<p>If you are a BU undergraduate and are starting a formal program in GMS (Masters or PhD), or are enrolled in a certificate program\u00a0 or as non-degree student, and had accommodations as a BU undergraduate, you must call Disability and Access Services (DAS) at 617-353-3658 and set up an appointment (remote or in-person).<\/p>\n<p>You must request to have your Dean&#8217;s letter with approved accommodations sent to the GMS liaison as GMS does not follow the same process as the CRC.<\/p>\n<p>Once GMS receives your Dean&#8217;s Letter approval, you will receive an email requesting completion of the <a href=\"https:\/\/www.bumc.bu.edu\/gms\/students\/dei\/gms-disability-services\/arf\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"color: #0066cc;\">GMS-Accommodation-Release-Form (ARF)<\/span><\/a> enabling GMS to notify your faculty. You are encouraged to complete this as soon as possible prior to the start of class to be sure accommodations are in place.<\/p>\n<p><\/div>\n<\/div>\n\n<div class=\"bu_collapsible_container \" aria-live=\"polite\" data-customize-animation=\"false\"><h4 class=\"bu_collapsible\" aria-expanded=\"false\"tabindex=\"0\" role=\"button\">Transferring Accommodations from GMS to GSDM\/CAMED<\/h4><div class=\"bu_collapsible_section\" style=\"display: none;\"><\/p>\n<p>If you are a master&#8217;s or PhD student graduating and starting in another program at BU (Medical, Dental, PhD, etc. ) and had accommodations as a graduate student, you must call Disability and Access Services (DAS) at 617-353-3658 and set up an appointment (remote or in-person).<\/p>\n<p>You must request to have your approved accommodations sent to your new school.
