{"id":8,"date":"2007-12-20T15:35:42","date_gmt":"2007-12-20T19:35:42","guid":{"rendered":"https:\/\/www.bumc.bu.edu\/ghbusm\/setupelective\/forms\/"},"modified":"2025-05-29T11:20:29","modified_gmt":"2025-05-29T15:20:29","slug":"forms","status":"publish","type":"page","link":"https:\/\/www.bumc.bu.edu\/ghbusm\/setupelective\/forms\/","title":{"rendered":"Forms for 4th-yr Student GH Electives"},"content":{"rendered":"<p>The following is a list of forms for fourth-year students who will undertake a GH elective.<\/p>\n<p><strong><i>Before<\/i> Elective is Undertaken:<\/strong><\/p>\n<ul>\n<li><a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/files\/2012\/05\/add_form_GH.doc\"><strong>ADD Form for GH Elective<\/strong><\/a> (to add an GH elective to your fourth-year schedule)<\/li>\n<li><a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/acknowledgement-and-waiver-of-liability\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Liability Form<\/strong><\/a> (all students who undertake an GH elective or summer project must submit this form online.)<\/li>\n<li><a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/files\/2012\/10\/Liability_Form_Countries_with_Travel_Warning.doc\"><b>Liability Form for Countries with Travel Warning<\/b><\/a> (students who wish to pursue an elective or summer project in a country with a travel warning must print,<br \/>\nsign, and return form)<\/li>\n<li><span><strong><a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/travel-grant\/\" target=\"_blank\" rel=\"noopener noreferrer\">Travel Grant Application<\/a><\/strong> (You may use this form to apply for funds for a GH elective. Travel grant applications are processed on a rolling basis with a funding determination sent to the student within 3 weeks of submission.)<\/span><\/li>\n<\/ul>\n<p><strong><i>After<\/i> Elective is Undertaken:<\/strong><\/p>\n<ul>\n<li><span>Complete the GH Post-Experience\u00a0Report at\u00a0<a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/global-health-post-experience-summaries\/\"><strong>Global<\/strong><br \/>\n<strong>Health Post-Experience Summaries<\/strong><\/a>. Once you log into\u00a0<a href=\"https:\/\/www.bumc.bu.edu\/ghbusm\/global-health-post-experience-summaries\/\">Global<br \/>\nHealth Post-Experience Summaries<\/a>, click &#8220;Click here to submit an experience&#8221;<br \/>\nto submit your summary report.\u00a0This must be submitted within 30 days of student\u2019s<br \/>\nreturn and no later\u00a0than the Friday before graduation.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The following is a list of forms for fourth-year students who will undertake a GH elective.
