To disclose the speaker\u2019s relevant financial relationships in section 1, click \u201c<strong>create new<\/strong>\u201d. Answer all questions required, and click \u201c<strong>save<\/strong>\u201d. All relevant financial relationships must be reviewed by the course director to determine if a conflict of interest exists. To \u201csign\u201d the Disclosure form in Section 4, input the date the disclosure form was completed by the speaker. Click \u201c<strong>submit<\/strong>\u201d.
