MASBIRT TTA is funded by the MA Department of Public Health, Bureau of Substance Abuse Services (BSAS).<\/p>\n<p>MASBIRT TTA\u00a0supports BSAS\u2019 ongoing efforts to establish SBIRT practice as a standard of care in diverse healthcare settings throughout Massachusetts by providing implementation and sustainability guidance, along with skills training and coaching for all levels of clinical and administrative personnel. MASBIRT TTA uses diverse teaching methods and\u00a0ongoing technical assistance, troubleshooting, and coaching. The MASBIRT TTA\u00a0team also utilizes\u00a0SBIRT expert consults to assist with training and technical assistance, as needed.<\/p>\n<p>The MASBIRT TTA team has provided local, regional and national SBIRT training and education as part of the Substance Abuse and Mental Health Services Administration (SAMHSA) funded state SBIRT cooperative agreement from 2006-2012. The MASBIRT TTA team also directed the successful implementation of universal SBIRT services in diverse clinical settings (i.e., inpatient, outpatient, emergency\/urgent care, specialty healthcare settings) for over 170,000 patients.<\/p>\n<h3><strong>What is SBIRT?<\/strong><\/h3>\n<p><span style=\"font-family: HelveticaNeue-HeavyCond;\"><span style=\"font-family: HelveticaNeue-HeavyCond;\">S<span style=\"font-family: HelveticaNeue-Condensed;\">creening, <\/span><span style=\"font-family: HelveticaNeue-HeavyCond;\">B<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">rief <\/span><span style=\"font-family: HelveticaNeue-HeavyCond;\">I<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">ntervention and <\/span><span style=\"font-family: HelveticaNeue-HeavyCond;\">R<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">eferral to <\/span><span style=\"font-family: HelveticaNeue-HeavyCond;\">T<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">reatment (SBIRT) is a comprehensive, <\/span><\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">integrated, public health approach to identify patients whose patterns of alcohol and\/or drug use put their health at risk and intervene early.<\/span><\/p>\n<h3><span style=\"font-family: HelveticaNeue-Condensed;\">SBIRT components are:<\/span><\/h3>\n<ul>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">Universal annual S<span style=\"font-family: HelveticaNeue-HeavyCond;\">creening <\/span><span style=\"font-family: HelveticaNeue-Condensed;\">identifies unhealthy use. 75-85% of patients will <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">screen negative. For those who screen positive, further assessment is needed to determine level of risk.<\/span><\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">\u00a0<\/span><\/li>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-HeavyCond;\">Brief Intervention\u00a0<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">provides feedback about unhealthy substance use. <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">It also focuses on education, increasing patient insight and awareness about risks related to unhealthy substance use and enhances motivation toward healthy behavioral change.<\/span><\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">\u00a0<\/span><\/li>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-HeavyCond;\">Referral <\/span><span style=\"font-family: HelveticaNeue-Condensed;\">to <\/span><span style=\"font-family: HelveticaNeue-HeavyCond;\">Treatment\u00a0<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">helps facilitate access to specialty assessment and <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">addiction treatment for those who need specialized care. A referral is usually indicated for only about 5% of people screened.<\/span><\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">\u00a0<\/span><\/li>\n<\/ul>\n<h3><strong>Why is SBIRT important?<\/strong><\/h3>\n<p><span style=\"font-family: HelveticaNeue-Condensed;\">Unhealthy alcohol and other drug use are among the most common causes of preventable morbidity and <span style=\"font-family: HelveticaNeue-Condensed;\">mortality (Mokdad, <em>JAMA<\/em> 2004)<\/span><span style=\"font-family: HelveticaNeue-Condensed;\">. Despite their frequent presentation in healthcare settings, unhealthy alcohol and other drug use often <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">go unrecognized. While there is substantial research on the effectiveness of SBIRT\u00a0on alcohol use, there is less research on the impact of <span style=\"font-family: HelveticaNeue-Condensed;\">SBIRT\u00a0on other drug use.<\/span><\/span><span style=\"font-size: xx-small; font-family: HelveticaNeue-Condensed;\">\u00a0<\/span><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">Unhealthy substance use can complicate existing chronic conditions like diabetes,<span style=\"font-size: xx-small; font-family: HelveticaNeue-Condensed;\"><span style=\"font-size: xx-small; font-family: HelveticaNeue-Condensed;\"> <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">hypertension, <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">cardiovascular diseases or mental health disorders<\/span><span style=\"font-size: xx-small; font-family: HelveticaNeue-Condensed;\"><span style=\"font-size: xx-small; font-family: HelveticaNeue-Condensed;\"> <\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\">and interact with prescribed medications.<\/span><\/span><\/span><\/span>Research has shown that large numbers of people whose patterns of use put them at-risk of developing <span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">alcohol or drug problems can be identified through screening (Babor, <em>Substance Abuse <\/em>2007).<\/span><\/span><\/p>\n<h3><span style=\"font-family: HelveticaNeue-HeavyCond;\"><span style=\"font-family: HelveticaNeue-HeavyCond;\">SBIRT <span style=\"font-family: HelveticaNeue-Condensed;\">has\u00a0been found to:<br \/>\n<\/span><\/span><\/span><\/h3>\n<ul>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\">decrease the frequency and severity <span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">of drug and alcohol use (Madras, <em>Drug\u00a0Alcohol Depend <\/em>2009; Kaner, <em>Drug\u00a0Alcohol Rev <\/em>2009; Humeniuk, <em>Addiction <\/em>2012)<\/span><\/span><\/span><\/li>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">decrease emergency department visits and<\/span><\/span><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">\u00a0hospital days (Fleming, <em>Alcohol Clin Exp Res<\/em> 2002)<\/span><\/span><\/li>\n<li><span style=\"font-family: HelveticaNeue-Condensed;\"><span style=\"font-family: HelveticaNeue-Condensed;\">demonstrate net-cost savings (Fleming, <em>Alcohol Clin Exp Res<\/em> 2002;\u00a0 Estee, <em>Med Care\u00a0<\/em>2010; Solberg, <em>Am J Prev Med<\/em> 2008)<\/span><\/span><\/li>\n<\/ul>\n<p>The U.S. Preventive Services Task Force (USPSTF) recommends screening and behavioral counseling interventions to reduce alcohol misuse by adults, including pregnant women, in primary care settings. The National Quality Forum (NQF) endorses outpatient annual alcohol screening and recommends a\u00a0systematic approach be used to identify drug use. The Joint Commission currently has optional quality standards that require unhealthy substance\u00a0use identification and management in inpatient settings.<\/p>\n<h3><strong><strong>How will\u00a0practices have time\u00a0for SBIRT?<\/strong><\/strong><\/h3>\n<p>With once per year screening, most patients (between 75-85%) will be negative. Screening consists of 3-4 simple questions, requiring 2-3 minutes\u00a0of staff time. A full screening (e.g., assessment for consequences and severity of use) and brief intervention (i.e., brief, motivational counseling) for positive patients (15-25%)\u00a0takes between 5-20 minutes.
