These light chains are part of immunoglobulins, also called antibodies. But in this disease, light chains misfold and aggregate into amyloid fibrils that deposit in organs and tissues.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"alignleft\"><img loading=\"lazy\" width=\"180\" height=\"180\" src=\"\/camed\/files\/2015\/06\/Vaishali-Sanchorawala-1.jpg\" alt=\"Head and shoulders of Dr. Sanchorawala wearing dark suit jacket, dark frame eyeglasses, slightly smiling\" class=\"wp-image-80535\" srcset=\"https:\/\/www.bumc.bu.edu\/camed\/files\/2015\/06\/Vaishali-Sanchorawala-1.jpg 180w, https:\/\/www.bumc.bu.edu\/camed\/files\/2015\/06\/Vaishali-Sanchorawala-1-150x150.jpg 150w, https:\/\/www.bumc.bu.edu\/camed\/files\/2015\/06\/Vaishali-Sanchorawala-1-100x100.jpg 100w\" sizes=\"(max-width: 180px) 100vw, 180px\" \/><\/figure><\/div>\n\n\n\n<p>In a <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMra2304088?rss=searchAndBrowse\">review article<\/a> of AL amyloidosis \u201cSystemic Light Chain Amyloidosis,\u201d <a href=\"https:\/\/profiles.bu.edu\/Vaishali.Sanchorawala\">Vaishali Sanchorawala<\/a>, MD, director of the Amyloidosis Center at the Chobanian &amp; Avedisian School of Medicine and Boston Medical Center, focused on recent advances in the understanding of pathogenesis, clinical syndromes, risk stratification and therapeutic advances, and looking at future efforts and needs in treatment and research.<\/p>\n\n\n\n<p>\u201cThe care of patients with systemic immunoglobulin light chain (AL) amyloidosis has undergone transformative changes, leading to marked, steady progress in outcomes for patients over the past four decades,\u201d Sanchorawala wrote in her review. \u201cOverall survival has improved considerably, yet many unmet needs persist.\u201d<\/p>\n\n\n\n<p>\u201cOne of the most important determinants of survival is the severity of cardiac involvement,\u201d said Sanchorawala. The heart is affected in 70 to 80% of patients with AL amyloidosis and cardiac problems are the leading cause of death, but other major organs like kidneys, liver, and peripheral and autonomic nervous systems can also be affected by this disease.<\/p>\n\n\n\n<p>Therapies include those targeting clonal plasma cells, stopping light chain production, and new research into antifibril monoclonal antibodies that accelerate the removal of amyloid deposits from the organs.<\/p>\n\n\n\n<p>\u201cThe arsenal of therapeutics for AL amyloidosis is rapidly expanding, offering a promising outlook and emerging triumph over adversity for patients in 2024,\u201d said Sanchorawala.
