Department of Psychiatry Morbidity & Mortality Series: Managing a 15 Year-Old Girl with a History of Trauma, Severe Mental Illness, Substance Use, and Multi-System Involvement

The goal of the Boston University Department of Psychiatry Morbidity & Mortality Series is for trainees to present challenging cases that create an opportunity for the department to consider improvements to quality of care for our patients. We seek to learn from past cases and apply this learning to future patients and systems of care. In this particular M&M, we will discuss the case of a 15 year-old girl cared for in outpatient child psychiatry, who has a history of trauma, severe psychiatric symptoms, substance use, and multi-system involvement. We will discuss the escalating severity of her presentation over time, consider ways in which we can prevent the progression from trauma-related psychiatric symptoms to SUD, and consider how we can effectively work with community providers and systems to best care for patients. 

Learning Objectives:
    1. Describe ways in which early care could be provided in an effective way for at risk youth to prevent escalation and development of severe substance use disorders
    2. Identify gaps in care that can occur despite involvement of multiple systems
    3. Discuss ways that multiple systems can better coordinate to serve an at-risk child or adult
