SPRING 2007 APPLICATION INSTRUCTIONS for NEW STUDENTS

IMPORTANT NOTES

International students are not eligible for institutional or federal financial aid. Please see our International student Preferred Private Lender/Product Options, or call 617-638-5130 or 877-776-6243 for more information.

Incomplete applications delay processing and may result in reduced institutional funding. Please be sure all required documents are attached when you submit your application.

The Office of Student Financial Services (OSFS) DOES NOT accept faxed application material.

You may want to print these instructions to help with the process.

PLEASE NOTE: Many of the documents are in IPDF format and allow you to type your information directly on the material and print it. These documents require Adobe Acrobat Reader to view and print. The Reader can be downloaded for free from Adobe.com. You may tab between fields, or click over a field. Please be sure to exit all fields before printing, and once printed, please remember to clear all data.

APPLICATION DEADLINE AND DOCUMENTS REQUIRED

All documents and results required for the BUSPH Spring 2007 Application for Financial Assistance for New Students are due by 6:00 p.m. January 31, 2007 or within 45 days of admission, whichever is later. We suggest filing FAFSA and PROFILE as soon as possible. NOTE: The last day to complete PROFILE on-line with the CollegeBoard is December 10, 2006.

Required of all institutional aid applicants. NOTE: Please be sure all student statements are signed before submission.

*File Free Application for Federal Student Aid (FAFSA)
Required of all institutional and federal aid applicants. NOTE: Institutional aid applicants, please complete the student sections only.
Federal loan only applicants, please complete only the student sections and the William D. Ford Direct Loan Request Form.

*Student 2005 Tax Information
Required of all institutional aid applicants.
Attach to your BUSPH application:
A SIGNED copy of your/your spouse’s 2005 federal tax return (1040, 1040A, 1040EZ, 1040TEL)
Copies of all schedules filed with your federal tax return
Copies of all W2s filed with your federal tax return (total should be equal to the wages line on the 1040)
OR
Student Non-Filing Statement (non-filers only).

Required of applicants (or spouse) who own or invest in a sole proprietorship, a partnership or limited partnership, an S-corporation, a corporation, or a farm.
Business Farm Supplement and the following FEDERAL tax documents as applicable:
Sole Proprietorship – none
Partnership or Limited Partnership – 1065 and schedules K1 for all owners/investors
S-Corporation – 1120S and schedules K1 for all owners/investors
Corporation – 1120
Farm – none

*William D. Ford Direct Loan Request Form
Required of all federal loan applicants.

*Citizenship Documentation
Required of new students who are naturalized U.S. citizens or permanent residents.
Naturalized citizens, attach a notarized copy of your U.S. Passport or Certificate of Naturalization.
Permanent residents, attach a notarized copy of the front and back of your alien registration (green) card.
