Instructions

Descriptions of different formats, their system requirements, and how they should be used:

IPDF: Interactive Portable Document Format by Adobe An IPDF is interactive; i.e., it allows a user to enter information into predesignated fields in the form. In order to use IPDFs, you must have Adobe Acrobat Reader 4.0. You may download Adobe Acrobat Reader 4.0 for free by clicking here. To use the form, simply place the hand-shaped mouse pointer over a field. If the field is a text field, the hand-pointer will turn into an I-beam; click on the field and type the requested information. Fill in check-boxes by placing the hand pointer over the check-box and clicking with the mouse. After you have finished entering all requested information, print out the form using the print button on the Adobe Reader toolbar. Whenever you close an IPDF, all your entered information will be lost, so do not close an IPDF until you are satisfied with the printed output.

Microsoft Word Form A Microsoft Word Form is a word document that has had its general formatting protected; a user may only enter information into predesignated fields. Use the Tab key to navigate through the form’s fields and type to enter text into text fields. To put a check into a check-box, either click the box with your mouse pointer or highlight the check-box and press the X key. You must have Microsoft Word 97 or a later version to use the form on this page. Unlike IPDFs, you may save Microsoft Word Forms with your responses to your hard drive.

Microsoft Word Document A Microsoft Word Document does not have protected formatting. A user may enter information electronically into the form but the format (such as underlines and spacing) will likely be corrupted; for this reason it is recommended that you print the form and enter the information by hand. Use this version if you do not have at least Microsoft Word 97 or if you use another word-processing package like Corel WordPerfect.
