AHA Tobacco Regulation and Addiction Center (A-TRAC) Trainee Application

Because THIS FORM DOES NOT ALLOW YOU TO SAVE your work and return to it, you may want to write your answers in a separate document and then paste them in the application to avoid the risk of losing your work.

  • (A) Please provide a one page personal statement: (1) Please describe your interest in tobacco-related research, education, or advocacy. (2) Please describe how your background prepares you for the training project. (3) What specific skill sets to do you hope to develop through your participation in A-TRAC? (4) What are your long term career goals and how will participation in the two-year program enable you to better achieve them? (B) Please list the specific tobacco research project(s) you would be most interested in if you are selected for the fellowship.
  • (A) Please send a transcript from your graduate or professional school, if relevant. (B) If you are a physician, please send USMLE scores.
  • Please provide the following project information in 2-3 pages: (1) Title; (2) Objectives; (3) Hypothesis; (4) Specific Aims; (5) FDA Interest Areas that your project addresses; (6) Significance; (7) What challenges do you foresee to completing your project? How will you address these challenges?); (8) Project timeline and milestones; (9) References cited. Please also include any regulatory procedures (IRB, IACUC, etc.) that will need to be in place before the project commences.
  • Please provide the name, address, telephone number and email address of three individuals who will send reference letters on your behalf. Please indicate your relationship to them. Please ask your references to email their letters to Abigail Russo at arrusso@bu.edu.
