Submission Center IH Summer Scholarship Applications, 2013

To apply for a International Health Summer Scholarship (2013), you must submit the following:

ITEM #1: Online Application for IH Summer Scholarship (the form below)
ITEM #2: IH Summer Scholarship Proposal (Word document) to be submitted below and named as: 'Your Last Name, Your First Name, IH Summer Scholarship Proposal, 2013.doc'. Click here for the document: IH Summer Scholarship Proposal.doc.
ITEM #3: E-mail from Preceptor. The person who is supervising your must forward an e-mail to Ana Bediako indicating the extent and manner of support provided by the sponsoring organization/individual, what your role will be in the outlined project, and who will be directly supervising you. {This does not apply to the following programs: CachaMSI, Cinterandes, or Interhealth.}
ITEM #4: Liability Form. You must print, sign, and deliver the B.U. liability form (paper document) to Ana Bediako. If Ana is out, please slide under the door.

Before you begin, you must read the following document: IH Summer Scholarship, 2013 - Description & Requirements.

Note on IRB or IACUC Approval: If the proposed project is research and it involves humans as research subjects or the use of vertebrate animals, IRB (Institutional Review Board) or IACUC (Institutional Animal Care and Use Committee) approval is required for the project to be funded. The student or preceptor must provide the appropriate documentation (proof of approval by IRB or IACUC) by June 1 at the latest to Ana Bediako. If you do not have IRB or IACUC approval, you cannot start your project.

DEADLINE: All parts of the application must be submitted by Friday, April 5, 2013, 11:59 PM.
