Application Form

The application process for the July 2011 start date is now CLOSED.  If you would like to start preparing your application for the July 2012 start, the following must be provided in order for your application to be considered:

1. Completed application form
2. TOEFL scores sent directly from ETS (center code 3231)
3. Three letters of recommendation (one from your current supervisor)
4. Personal Statement
5. Application fee of $100 USD made payable to Boston University
6. Current curriculum vitae
7. Notarized copy of medical degree translated into English
8. Medical school transcripts, either notarized or sent directly from the medical school
9. Financial sponsorship documentation

The application deadline for the July 2012 start date will be November 15, 2011.
