CRIT Program Faculty Mentor Online Application

Faculty Mentor candidates must:

  • Be a Program Director or core residency program faculty
  • Have a chief resident who has applied to the 2015 CRIT program
  • Be able to assist with the development and implementation of the Chief Resident Substance Use Teaching Project
  • Be able to attend the entire training
  • Complete the Faculty Mentor CRIT Application

14th Annual Chief Resident Immersion Training (CRIT) Program: FACULTY MENTOR Application

Application materials will be accepted until February 6, 2015. Complete the application below, including uploading of required documents.

Your incoming Chief Resident must be accepted into the CRIT program in order for you to attend. With limited faculty spots, selection of faculty attendees is based upon your statement and faculty position.

To complete the application process, we require: 1) a CV and 2) a statement (300 words or less) which incorporates a) how attending the CRIT Program with your Chief Resident will enhance his/her ability to implement addiction medicine curriculum including completion of a Substance Use Teaching Project; developed at CRIT and b) how your current faculty position will enable you to add addiction medicine content into the residency program curriculum. Note: This statement will serve as the letter of support for your Chief Resident’s application.

The CRIT Program is funded by a grant from the National Institute on Drug Abuse (NIDA). You will be asked to complete several surveys, which we will send to you. Your survey will not be available to program applicant reviewers and will not influence whether or not you are accepted. Your application will not be considered complete until you have returned the pre-course survey.
