Annual Faculty Evaluation Form
One of the recommendations of the Ad Hoc Committee on Faculty Development in 2005 was:
“In addition to the annual report, a faculty development form should be provided to the faculty member regarding career goals and areas and accomplishments, concerns or other issues not contained in the annual report. This should include, but is not limited to, the ten points of information found in Attachment 1.”
