DOSIMETRY BADGE & RING REQUEST (all fields are required)

Individuals meeting one of the following criteria are required to wear a dosimetry badge and ring:

  1. Workers handling quantities greater than or equal to 1 mCi of external hazard beta/gamma emitters
    (P-32, I-125, I-131 or Cr-51).
  2. Workers routinely involved in radiographic, fluoroscopic or x-ray diffraction procedures.
  3. To determine if you need a dosimetry badge, please refer to this attached Dosimetry Requirement Decision Tree.
