User Guide for On-line Work Order Requests

This guide contains two sections to assist Boston University Medical Campus users in submitting online Facilities Work Order requests.

Explanation of Online Work Order Request Fields
Field Name Required/Optional Explanation
Requester’s Name: Required Entry Enter the name of the person making the request or the person who will act as the department contact for this request.
Requester’s Department: Required Entry Enter the requester/contact department name.
Requester’s UID#: Optional, but Preferred Enter the requester’s University ID Number. Work order requests will only be processed for Boston University and Boston Medical Center staff.
Phone Number: Required Entry Enter the requester/contact work phone number.
Email Address: Required Entry Enter the requester/contact email address.
Site Affiliation: Pre-entered by System The work order system will default to “BU Medical Campus”. Do not change this selection.
Building: Required Entry Select the building in which the work is to be completed from the pull-down menu. If work is to be completed outside of the buildings on this list or is work that must be completed between different locations, select “Other” and provide details in the Room or Location Description field below.
Floor: Required Entry Select the floor on which the work is to be completed from the pull-down menu. If you selected “Other” in the Building field, select “Other” in the Floor field as well and provide details in the Room or Location Description field below.
Room or Location Description: Optional Provide room number or additional, detailed information about the location at which the work is to be completed.

The first accounting line is required. The second and third are optional.

Enter at least one Unit, Department, (Source if applicable), percent combination to indicate the account that is to be charged for this work. All Facilities work orders are charged to object code 0931; therefore, it is unnecessary to include this part of the accounting line. If only one account is to be charged, the percent will always be 100.

If multiple accounts are to be charged, enter the appropriate unit/department/source/percent information for up to two more accounting lines.

  • Unit Codes are 3-number values.
  • Department Codes are 3-number values.
  • Source Codes are 5-number values containing a four digit source prefix and one digit source suffix.
  • Total of all percents charged should equal 100%.
Type of Work Requested: Required Entry Select the trade, custodial, or other area of Facilities Management that will need to complete the work.
Description of Work Requested: Required Entry Provide as much detail as possible about the work being requested in order to avoid delays or additional questions from Facilities Management.
Priority: Required Entry If you are only requesting an estimate and not the actual work, select “Request Estimate Only”. Otherwise, select the appropriate Work Order request entry: Emergency or Non-emergency.
Authorized Amount: Optional Enter the dollar amount that the requester is authorizing for this work. If requesting an estimate only, enter “N/A” (not applicable). If left blank, the Office of Facilities Management will complete the work based on their best estimate after funding has been approved.
Auth Amount Based On: Optional If you provided an Authorized Amount for this work, enter a brief explanation of how you arrived at that amount (e.g., written estimate from Facilities, telephone estimate from vendor, best judgment of requester, etc.)

Service Areas for Boston University Facilities Department

Boston University and Boston Medical Center collaborate in many operational areas. This is often the case with services provided by the Facilities departments of both organizations. However, due to accounting and other needs, work orders must be processed through two separate systems. The chart below is provided to assist faculty and staff in using the correct work order system by indicating which areas are serviced by the BU Facilities departments. Of course, there are exceptions and special arrangements that may result in an individual attempting to submit a work order through the wrong system, but when this happens, staff at the Facilities departments will contact the requester and redirect them to the appropriate work order request system.
