New Patient Information
We appreciate your selection of this office to serve your medical care. We are committed to providing you the best care available. To keep the cost of your care to a minimum, we will work together and ask that you read the following information. We look forward to serving you and your family.
Office hours
Our offices will be open Monday thru Friday from 8:30am-4:30pm. The answering service will answer calls between 12:00-1:00 pm and after 4:30 pm. If you have an emergency they will get in touch with the office or the physician.
Referrals
If your plan requires referral from your primary care physician (PCP), please call them before your visit. We are required by your insurance company to have this referral prior to the visit. We will be glad to re-schedule your appointment if needed.
Payment Policy
All co-payments are due at the time of service. We do not bill for co-payments. We accept checks, cash, Visa, MasterCard, American Express & Discover. Any questions regarding your account should be directed to our billing office at 1-800-478-6675.
Late patient
We will do our best to accommodate patients who arrive a few minutes late for their appointments. It maybe necessary to re-schedule your appointment depending on the physician’s schedule.
Returned Checks
A fee of $20.00 will added to the patient account for all returned checks
No Show
Patients who do not show up for the appointment without notification may be subject to a no show fee of $20.00.
At our Boston facilities the policies of Boston Medical Center will apply.
